FDA rejects new diet drug

The U.S. Food and Drug Administration has rejected the approval of Qnexa, an experimental drug used to treat obesity.

The FDA told the drug maker, Vivus, that the drug could not be approved in its present form, the company said Thursday in a statement. Qnexa is a combination of two older medications: phentermine, an amphetamine that made up the "phen" half of Fen-Phen, the diet drug pulled from the market in the 1990s after some users developed heart problems. The second component of Qnexa is topiramate, an anti-seizure medication that's been used to treat a range of conditions, including alcoholism.

The FDA's decision does not come a surprise. In July, an FDA advisory committee recommended Qnexa not be approved, citing concerns ranging from heart issues to psychiatric side effects. The company says it is not going to give up. "We remain confident in the efficacy of Qnexa and look forward to continue working with the FDA towards the approval for the treatment of obesity," said Leland Wilson, chief executive officer of Vivus.  The company plans to respond to the FDA around mid-December.

    How about instead of tha magic diet pill, you employ some self discipline and go to the gym.
    All these people with a million excuses as to why they are over weight. Discipline, dedication and willpower will prevail EVERY time over a magic pill anyway. Excuses only give you a crutch to avoid the change you claim to want.

    I like "I'm fat because I eat unhealthy because I don't have the money to eat healthy" because it makes no logical sense. You're poor so you eat more calories? Just eat half the cheeseburger.... Sure, McDonalds is cheaper. But that doesn't force you to over eat.

    I agree most of it is lifestyle, even if someone has genetics that tend to make them heavier, 90% is lifestyle choices. People who establish strong patterns of healthy eating habits & regular exercise have far less risk of being overweight/obese. However, if you are prone to being overweight genetically and you are older it's certainly an uphill battle. The slowing of the metabolism with age, combined with hormone deficiencies and less efficient mitochondria and other cellular processes make staying lean more difficult at 50+

    Amber, sorry...there aren't that many bad thyroids floating around out there. If you take in more calories than you burn, you will gain weight. If you do this continuously you will become obese. Eating right and exercising is all that is needed.

    Just like "acid reflux disease"; stop eating horrible things and drinking tons of caffeine/beer and or smoking. Just take these pills and you can eat all the horrible things you want and not gain weight or get heartburn...and then when your cholesterol gets out of control you can take lipitor and keep on rolling through that McDonalds drive through...

    Tyler, I completely disagree. I am one that says that over-eating is the cause 99.999% if the time (the other .0001% are the rare medical anomalies) and I was obese for about eight years. I lost the weight, and am keeping it off. As a reflective measure, I see how much my habits have changed. I am very active and watch my caloric intake closely. The reason diet pills generally "don't work" is for the exact reason they didn't work for you - you may lose weight, but if you don't address the reason you gained it in the first place, it won't stay off. I have a friend who had thyroid cancer and she began to gain weight like crazy, blaming in on the cancer. After the problem was addressed and the medication regulated, she still claimed she couldn't lose weight. I was with her a lot and saw her consume mass quantities of caloric beverages, fried foods, junk food, etc. I was with her when her doctor finally told her point blank there was not a medical reason for her not losing weight at this point. Guess what, she finally restricted her calories, started exercising and has lost most of the weight.

    Absolutely. These fat oafs pretend it's genetic, glandular, blah, blah. B.S. You cannot created something from nothing. It takes 3500 calories to = 1lb of fat so these people have to socking away HUGE amounts of food and/or they are completely sedentary and no genetic-based excuses will alter those FACTS.

    Someone, I'm sure over-eating is a big cause but the impulse to eat I would say is stronger in a larger person. I've gotten my daily intake of calories to around 1200-1500 a day. I do brisk walks nearly every day and some other excercies when possible. The biggest problems I had trying to get out of my old routine was the impulse to eat and the energy to excercise. The weight loss pill gave me the energy and helped keep the hunger pains off. It can get pretty brutual. It's obvious that the pills were going to drop me below my real tolerance level but I've never been back up to the highest amount I was before I was on them. I was 260 and am currently at 235 (although, I was at 245 for awhile) but at one point I reached 200 when on the pills.

    I had cancer in the past and was extremely skinny when I was younger and forced to eat because of the drugs. Once all was said and done I was overweight. So, I'd say the chemotherapy did some of it but I do blame my lifestyle too. Although, I'm currently trying my best and taking vitamins daily. Blaming just over-eating seems like your trying to simplifying a much more complex problem.

    A lot of people who overeat do so b/c they are actually dehydrated. Drink 2-3 cups of water anytime you feel hungry or snacky, if you are still hungry 20 minutes after that then you can eat. From working in the nutrition & supplements field for 10 years, and I can tell you from helping many people with obesity/weight problems that if you write down everything you eat for one week(snacks/nibbles/drinks/water intake included) and bring it to a nutritionist or a personal trainer, they can tell you what you are doing wrong and how to correct it. Also, consider getting tested for food allergies, b/c they can be a major factor into why you sometimes have a hard time losing weight. Everyone who is obese/overweight can lose weight, its just a matter of educating yourself, will power, and finding a healthy lifestyle program(not a "diet") that works well with your schedule. Have a bowl of frozen mango slices for dessert instead of mango sorbet, order a kids meal at fast food restaurants to cut the portions & calories to less than half of the adult sizes, there are many things you can do that will make it easier to obtain a healthier weight.

    It sounds to me like it's the equivalent of taking adderall and topamax. Adderall definitely suppressed your appetite & is pretty similar to speed. It's also highly addictive. I think that if it's truly unsafe..the FDA will not approve it ever (this isn't the end all be all). The FDA has learned it's lesson & is gas really learned the hard way they have to be stricter. They've denied several meds in recent year or 2 that may gave been approved 10 years ago. They're stepping it up.

    @ Amber and all others on this string... I have PCOS. It is NOT caused by obesity as @Amber2 suggests but rather a symptom of the syndrome. If you are more overweight- it can cause a mulititude of other symptoms to occur such as hair loss, hair gain on your face, infertility, diabetes and whole bunch of other fabulous side effects for a woman to have to deal with. I would like to say that overeating is an addiction and just like any other addiction- people sometimes need help to overcome it. Whether that is a Weight Watchers meeting or a (SAFE) pill to help them get started- so be it. YES- taking in fewer calories can certainly help- but lets not forget that people do not go to bed at night skinny and wake up fat. It takes along time to change the habits. For anyone who has PCOS- starting a low carb diet is the best way to kick start your weight loss because it regulates your insulin and fights against the insulin resistance that many of us have that causes us to gain more weight. After that- I would love to suggest Weight Watchers. I lost 30 lbs in 6 oths and the habits I learned have helped me to keep it off. I would like to also say to all of the people assuming these people are fat and lazy- I'm not fat- I just have more compassion than most of you do. Easy to be VERY judgemental and hide behind your computer when calling people out that you do not even know. Try helping someone instead of putting them down next time. Stay classy!

    I don't understand why they keep trying to mix all these other drugs with phentermine... Phentermine is one of the oldest around, it works, and for the most part is extremely safe... What – it does't work FAST Enough??? Why do they keep finding it necessary to mess with it? The sad thing is that any of these things are just a crutch. As soon as you stop, you start gaining again. No drug will take the place of changing your lifestyle, which is the ONLY way you are going to change your weight...

    I, too, don't get the whole 'it's not my fault' mentality. If you educate yourself and put in the time and effort you CAN lose weight! You simply need to know your BMR and know how many calories you are taking in. Take in less calories than you burn in a day and you will start to lose weight. Period! One pound is the equivilant of 3,500 calories so – If a person burns 2,000 calories a day but only takes in 1,500, then he or she will lose about a pound a week! Throw in a little exercise and it not only burns even more calories, but it also speeds up your metabolism.

    A healthy lifestyle is not hard to acheive, but it does require change and education. You HAVE to do your research and you HAVE to apply what you have learned. Most people I run into think because they eat salads and sandwiches all the time that they should be losing weight, but they don't take into consideration what is ON those salads and IN those sandwiches. Instead of focusing on what you're NOT supposed to eat, be more aware of what you are putting in your mouth and it's NUTRITIONAL VALUE. Not only will you feel good physically but you will also feel good MENTALLY!!!

    A magic 'diet' pill is not going to fix anyone's problem because it doesn't get rid of the cause, only the result.

    It facinates me that the FDA actually declined to approve a drug with all the crap that they have actually approved! Maybe it really works and the diet industry bribed them not to approve because they'd lose their multi-billion $$$ year scam revenues.
    However, as someone who takes topamax for migraines, I can't imagine taking it with phentermine...the side effects of topamax alone suck and I can't wait until I can go off of it! (and it is NOT a miracle weight loss pill. I have lost a few pounds since I've been taking it but mostly because it makes sweets and diet sodas taste weird)

My doctor couldn't figure out why I couldn't lose weight despite an 800 calorie medically supervised diet, 6 day a week exercise regimine (cardio and weights). I guess she wasted all her money on medical school...she should have just checked here with all of you experts.

      Genius, it is physically (in the "physics" sense) impossible to be eating an 800-Cal diet (with no cheating) and be working out aerobically and not be losing weight. Something is wrong somewhere unless you happen to be 3'3" tall. One does not need to go to med school to know this. It is like saying that you spend twice as much money as you deposit in your bank account and yet your balance is staying the same. Impossible.

    I've never figured out the obese due to metabolic problem argument in drugs. Some science 101. If you are diabetic, something isn't working inside you and you need a drug. If you stop eating, in theory, that would work. Does that treat diabetes? no.

    An obesity drug really is to treat over eating: they function by overloading your sense of full or blocking food absorbion. If you have a metabolic problem, you need a drug to fix your metabolic problem. If you can't metabolize food properly, WE SHOULDN"T GIVE YOU A DRUG TO STOP YOU FROM EATING.

    Yes, Amber, 99.9% IS from taking in more calories than are burned for fuel. Since humans share the same physiology ever wonder why societies where the predominant diet is fish & people get about on bicycles had a lean population? Being American doesn't confer a bunch of 'I can't help it medical condition'
    else we'd see the same thing in every country. As of note those who immigrate from Asian countries remain thin if they stay with their own dietary habits. If they take up Americanized eating habits they get fat. I agree with notfat, we do way too much 'accomodating', no one would think of making it easier for alcoholics/addicts with 'acceptance' groups et al which is why many of them make the effort & get clean & sober; the overeating excuses do not fly...denial isn't a river in Egypt.

      The second half of the equation is exercise/metabolism. That's what almost all the bullies on the thread are getting wrong. You can keep cutting your intake but if your metabolism is screwed up, it won't matter and the more your cut, the more your body will try to conserve as it protects itself.

      TJ, you just contradicted yourself. Exercising increases your metabolism. It strengthens your heart, muscles and makes your body a more efficient machine. So yes, if you are STARVING yourself, your body will go into conservation mode and you will hit a plateau, but if you eat a healthy diet and get plenty of exercise, it will benefit you in the long run.

      Before you start calling people morons, you should try doing a little research, educate yourself, then put your 2 cents in.

    Of course the FDA disapproved the drug. I guarantee that there have been diet pills rejected that could legitimately cause weight loss without exercise, but keeping to a balanced diet. Fat people are the health care system's bread and butter right now. Why keep them out of the hospital with this drug when they can make so much more money if everyone is buying a steady supply of insulin, and constantly being monitored for heart conditions?

    Yes obesity comes from eating more than the body expends. Some liver disease comes from drinking alcohol. Heart disease, diabetes and numerous other illnesses can be a direct result from the persons actions or lack there of. But there are meds for those illnesses i listed and society seems fine with it but why or why is it so hard to accept that there needs to be meds for those that do not control their eating.

    I totally agree with the other lisa.

    I guess all of you that seem to be scared of overweight people and talk so badly about them have no issues of your own! Don't throw rocks or your glass house may tumble! Every person has something that they could do better! Geez- get off your high horses!

    I agree with the FDA and I believe this drug should never be approved – both ingredients are dangerous and can prove to be fatal. I personally developed severe breathing problems while on topiramate (brand name =topomax) (which I was taking to prevent migraines) . The severe risks are not worth the possible benefit ...of taking this proposed new drug. Additionally, a caveat to remember is that sometimes it does take a while before a patient begins to experience a severe reaction to a medication so beware before you throw any pill in your mouth!

    Not true. If you reduce your caloric intake too quickly, you kill your metabolism at which point, it won't matter. You can be on a reduced calorie diet and you'll still gain weight.

    To the other person in this thread who stated the primary cause of PCOS was obseity, wrong... again. PCOS is polycystic ovarian syndrome. It's cysts on the ovaries and it's not caused by obesity – it's the other way around. The disease is affected by hormones. Do some research. I know it makes you feel better about yourself to blame all health conditions on being fat but it's really not the case. All diseases are not related to obesity.

    If you add up the money you spend on Mcdonalds, you could buy a bunch of veggies and some chicken and make a huge pot of soup that would last more than a week. You could freeze some for later meals. You could also grown some of your own produce right in your own home. People need to explore options...instead of just caving in sooo easily. No one else cares about your health....no one makes money off of healthy people, only off of sick people. As far as it not mattering WHAT you eat, it DOES matter what you eat. If you have a correct portion size of something, but it is loaded with unhealthy stuff it will be sure to derail your progress. Ex. a bowl of brown rice as oposed to a bowl of white rice. 2 slices of white bread, 2 slices of ezekiel bread(which by the way contains NO FLOUR). It does matter.

      The irony is that patients are often the guinea pigs for drug companies via off label use. Just ask migraine patients who were given paxill and prozac in the 90s only to discover later that the drugs were ineffective in doing anything but putting weight on them and possibly making their migraines worse... oh and depleting their bank accounts.

      Chances are, patients were given these "obesity" drugs already it's just that because they weren't "approved", their insurance companies wouldn't pay for them. That's what the FDA blessing would have taken care of.

    to all you who think you know what PCOS is, research it before you judge. Obesity DOES NOT cause PCOS, it's a symptom of it. Pcos a hormonal imbalance that causes many issues in a woman's body. After my son's birth I followed weight watchers religiously and worked out constantly with only a few lbs lost but after i finished nursing and could start taking my medication again, the lbs dropped off! Don't lump everyone in the same boat, some people actually have a medical condition and some are just lazy.

      There are those of us who have been on these drugs that are attempting to gain FDA approval as diet drugs and we are not bullies. At the same time, we know the side effects of the drug are much too severe for this type of treatment. That's why we're against approval. Some of us have been taking these drugs for years and have had to give up things like college/grad school and driving. Is this really the sort of sacrifice people are willing to make for a diet drug? Are people willing to struggle with things like aphasia and the ability to read a book in order to fit into a size 6?

      Somehow this seems like an extreme sacrifice. See, I'm willing to make it because without the drug(s), I'm unable to get out of bed and go to work. But... if I had to chose between being smart and being a size 6, I'd chose smart every time. And while I know the bullies on this page are probably horrified with that prospect, I'd rather be fat, smart and rich than thin, stupid and poor.

    Dear All,
    I would never apologize to anyone for being in unbelievable shape, having a six pack, looking amazing with my shirt off, and loving myself. It feels too damn good! I'm not a bully, I don't make fun of people who are overweight, I merly feel sorry for them. To feel how I feel is not impossible. It just takes motivation and a lifestyle change. I was never obese but a little chunky six years ago. I decided it was a time for a change. I started eating correctly (5-6 small meals a day), healthier, and exercising at least five days a week. I went from 189 to 162. This was just to lose the fat so I could start to work on gaining lean muscle. I know weigh 183 with 5.5% body fat. I work over 40 hours per week and have a very busy social life but always find time to get my workout in. I feel better then I ever have and I can thank exercise and eating right for it. If you are not losing weight from eating correctly- you are not eating correctly. Try a changing your approach to your diet and keep on exercising. Anyone can do it, just motivate yourself and stick to it!!! Everyone has days when they don't want to go or be at the gym, but I guarantee after a good workout you will feel better about yourself and 1000000000 times better since you got that workout in. I see too many Monday, Tuesday, and sometimes Wednesday workout people at the gym (PS I love Thurs and Fri when they don't come so I don't have to wait on equipment). I guess my point is: Come up with a plan, execute your plan, and stick to your plan. Nothing in life is easy but when you reach goals along the way, nothing feels better!!

    It's no surprised that the obesity rate in the last couple decades has increased exponentially. I highly doubt it's because of a massive increase in hereditary reasons. I don't doubt that medicine could help with weight loss, but it teaches you nothing about how to maintain the loss. The best thing you can do for yourself is to get out and go exercise. Ride a bike. Go walk. Jog. Sign up for a gym membership. Use an elliptical machine. Portion control is the second one. Read the labels. Write down your foods. Opt for skim milk. And lastly, try your best to get 7 hours of sleep. It's important to get decent sleep. It can be hard for those on a heavy work schedule, but try your best. I'm 21, male, 5 foot 9 inches, I was 230 pounds in later March. Now I'm down to 185. I've lost 45 pounds. I look better, I feel better. I'm still not where I want to be, but I know I'm making progress. I literally feel lighter on me feet, and it's a nice feeling. I'm not here to discourage people. I want to help people. I'm sure many out there are obese and unhealthy and it can be a miserable existence. But if you want the weight off, I'm here to tell you: "Yes we can!"

    It can cause heart problems. So? If I remain fat I'll still get heart problems. And nobody tell me this bull about exercise and diet. I've been trying for years and it obviously doesn't work for me. Exercise and diet isn't always the answer just like medicine isn't always the answer. EVERYONE IS DIFFERENT! Maybe exercise and diet works for some people but not for me.

    I have a great diet drug. It's called self-discipline. It works every time. When will everyone realize that if you want to be and stay healthy you need to eat better food, not eat crap, and get some exercise? It's not that complicated people! The unfortunate truth is that everyone wants a magic bullet that saves them from any sort of discipline or patience on their part. Welcome to America....

    You know what, everyone that is saying all this negative stuff about overweight people, you are the reason why I am fat. Someone said it's as simple as putting on a pair of shoes and going on a walk? BULL! I can't walk out the door without someone pointing, staring, or yelling obscenities at me! Even if I'm walking or at the gym I get this treatment(when it's obvious that I'm TRYING to lose the weight). I'm pretty much a recluse now and only leave the house when I have to. I have gone to the doctor asking what should I do and they tell me "eat right, exercise" and I'm like "NO DUH!" It's not that I am not trying, so don't assume that at all. I have even gone to a dietitian and she told me NOTHING that I didn't already know. People look at me and assume that I'm a lazy slob that is doing nothing to try to change it. But they are wrong, I've been trying for years while I hear the whispering of "oh look at that fattie" all around me. It's very counter productive people! Instead of belittling me, encourage me! If I'm walking to the store it's obvious I'm doing something to try and change my weight/health so the commentary isn't helping.

      You want some encouragement Abbyka? Get back in the gym! You CAN do it! It is NOT impossible! I had the same fears when I went to the gym expecting to see all these fit, skinny, muscled bodies and me looking like I had just had a baby (which I had). But when I got there, guess what? They all looked exactly like me and I very quickly pushed past my whole 'only fit people go to the gym' stygma. I ENJOY working out now. It is my release, my time to get away from work and chaos. I no longer dread getting on the elliptical and I like to push myself harder every day! I've gone from a size 12-14 down so a size 7 and I couldn't feel better! But I had to really make some hard changed in my life and with a new baby it wasn't easy! I made mistakes along the way and slipped but I got back up and went right back at it. It is something you have to do for yourself because you deserve it and EVERYONE DOES DESERVE IT!!!

      Wow, Abbyka, I understand the pain and embarrassment you feel. I treat patients with your situation every day. By the time they come to my office, they have given up all hope. After the first one or two weeks on a medical weight loss, plan, the hope returns. Consider a new approach. Go to http://www.ASBP.org, and find a board certified medical weight loss doctor. The best way to lose weight when trying on your own failed is to get professional help. You wouldn't try to treat your own diabetes or cancer, so why not see a medical doctor that specializes in the management of obesity? Look, this is a very complex disease. If "diet and exercise" advise worked, we'd all be thin.

      In the evaluation of the right plan, make sure it includes ALL of the following:
      1. Complete nutrition. It's not just about the calories, but also about the types of calories (carbs cause weight gain and hunger and protein fills you up and increases metabolism).
      2. A plan to gradually increase activity to levels that will help you sustain the weight loss.
      3. Education-usually in groups.
      2. Accountability and followup. Long term followup. Sometimes, medications, such as phentermine are required both in the short and long term. Only a doctor can help you decide if this is right for you.

      Don't worry as much about what anyone else thinks. Only you can make this happen for yourself. We have all been ridiculed for something we are, have or look like. While it hurts, you just have to look deep and decide to take care of yourself. I am yet to find someone who can't lose weight with a comprehensive approach. It's not time to give up! Getting the right help is a gift to yourself that you deserve! Best of luck.

In my angst for geitntg back to the blogosphere I opted for sharing some fun first, rather than showing off my latest whatever.Daisy Buchanan's is the restaurant one of my school's old haunts in Boston. It's much fun to go back to our past to visit. I've tried to live there at times, but I get kicked back out to reality.

    I can't believe some of these comments.

    There is no magic pill and no "one size fits" all solution to obesity. Being active and eating enough healthy food to meet your calorie needs is good advice for everyone, but quit automatically labeling overweight individuals as "lazy" because you don't percieve them to be working hard enough. No two people are obese for the same reason(s). Society needs to quit regarding overweight people as subhuman to people of "ideal weight" and look at them as real people with real feelings and problems. (I'm looking at you Marie Claire...)

    Chronic vitamin D deficiency is the root cause of obesity.

    As higher caloric intake occurs, the human body is tricked into thinking winter is approaching because of falling vitamin D levels (vitamin D is stored in fat).

    This in turn generates a response which causes the body to release chemicals that result in higher fat synthises. If vitamin D levels never rise fat is never shed and the situation perpetuates itself.

    Raising vitamin D levels in obese persons has been shown to reduce body weight by .5 /lbs per ng/ml. This is long term weight loss as proven by the study.

    There are two drugs that are very effective at helping individuals achieve long term weight loss and both have long records of safety. One is bupropion and the other is naltrexone. The combination of these two with therapy that encourages a healthier lifestyle beat any other weight loss strategy. The idea is that these two drugs help you to focus on things other than food. As you adopt new behaviors and practice these behaviors other aspects of life will become more rewarding and food will no longer have such a command of focus. The key is to get engaged in life so that those non food rewards take the stage.

    Thanks for your posting. I would also like to remark that the first thing you will need to conduct is verify if you really need credit restoration. To do that you have got to get your hands on a copy of your credit profile. That should really not be difficult, because government makes it necessary that you are allowed to acquire one free copy of your actual credit report annually. You just have to request that from the right individuals. You can either look into the website for that Federal Trade Commission as well as contact one of the leading credit agencies immediately.
