Study: Weight-loss combo pill shows promise

Two existing drugs, in combination, have shown significant promise in promoting weight loss, according to a new report.

The drugs,  phentermine and Topamax, in combination with lifestyle and weight-loss counseling were associated with a 18-22 pound weight loss in trial participants, compared with a three-pound weight loss in patients who received counseling alone.


The drug also appears to have reduced other obesity-related indicators, such as blood pressure, cholesterol and inflammation levels.

By comparison, the only approved long-term prescription weight loss medication available on the market today, Xenical, is associated with a seven-pound weight loss when combined with diet and exercise.

The study was funded by the pharmaceutical company Vivus, which combined the two drugs into one pill called Qnexa, and petitioned the U.S. Food and Drug Administration for approval in October 2010. The Food and Drug Administration denied the request, asking for more safety data from the company before moving forward.

According to the report, there were specific concerns about each drug's safety individually; specifically Topamax, which is reported to cause some psychiatric and cognitive side effects and may cause clef palate birth defects if taken while pregnant.

According to the manufacturer's study, the two drugs, when taken together, and at controlled-release doses, were very safe. Participants in the trial reported minor side effects such as dry mouth and constipation, however at higher doses, some participants did complain about some psychiatric side effects.

Among the 2,487 study participants, nine became pregnant, and none of their children were born with cleft palates, the report said.

The drug could be a good option for some people, but it's most likely not a silver bullet, said Dr. Melina Jampolis,  CNNHealth's physician nutrition expert, who is not connected with the study.

"This might be a good tool in the short term to get someone motivated, and that's great," she said. "But unless it's paired with aggressive lifestyle changes, it may not be great in the long term."

Phentermine is commonly used as a short-term weight loss treatment, and was previously paired with fenfluramine to make the successful but now-discontinued weight loss drug Fen-phen. Fen-phen was removed from the market in 1997 amid concerns about the drug's heart risk, but phentermine has been used successfully for short-term appetite suppression since then with no major safety concerns.

Topamax is commonly used to treat migraines and seizures.

    If you cannot tolerate Topamax for migraines or seizures, you may wish to look into Zonegran (zonisamide) as an alternative, but ONLY if you and your doctor(s) determine that it is safe for you. Like many others here, I could not tolerate Topamax. I was put on it to help facilitate weight loss, but also to help with migraines as a secondary effect. Unfortunately, I became very "foggy" at only 75 mg, and experienced uncomfortable, painful tingling in the extremities plus the inability to taste carbonation and other foods. Zonegran is similar to Topamax but may not cause the same negative side effects. I take 300 mg of Zonegran and have lost weight, controlled my migraines and not had the fuzzy, foggy mind and only rarely tingling in my fingers. Like Topamax, Zonegran also works as a low-level mood stabilizer for those with depression and bipolar disoder. I have borderline personality disorder and I have found it at least neutral in terms of psychiatric effect and have not experienced the definite negative psychiatric effects I had on Topamax. (And I'm not a shill - I just wanted to let people know there are some alternatives. I don't think Topamax is a bad medication; I just think it works differently in different people.)

      Here is another miagraine mirical( to us anyway) My daughter in law now 30 suffered them since she was 16 to the point of sever vomiting. Dr after Dr treatment after treatment finally found an anser through her own daughters Dr. He perscribed low dose of ridalin and she has been miagraine free for 5 months. This comming from 2-3 sever headaches a month. Here is the kicker, she also lost weight. I agree to woth most here. I too strugle with my weight and have been here to there with it all. The only true answer is eat less move more:)

    This story caught my eye because I was prescribed Topamax 2 years ago to treat my debilatating migraines. My doctor informed me of the side effects including and I quote "anorexic-like weight loss". I am/was on 150mg per day. I lost 3 lbs that first month and literally that was all I have lost up to and including today. I've had other side effects mentioned previously (Dopamax–forgetfulness and numbness/tingling) but thev've all subsided and my migraines have gone away as well. This drug worked wonders for me, but there was NO extreme weight loss anorexic (not that I'd want that) or otherwise. (10 lbs would've been nice!) :)

      I am currently on Topamax as well for Migraine headaches. I have been taking it for over a month now and the only side effects I have noticed is a tingling in my feet and hands and an extreme dislike for all carbonated drinks (which really screws up my beer!) I have lost a little weight but not much. I think that mostly has to do with the fact that I don't drink the carbonated drinks anymore. I haven't noticed any changes in the way I feel at all "mentally". I really think it all depends on the person. All I know is that I have had fewer migraine headaches in the past month!

    I took Topomax for lower back pain (at age 44), and found it induced extreme anxiety, paranoia, suicidal thougts & extreme sleeplessness. Good luck to anyone who takes it, in any combonation.

      I had similar experiences with topamax.
      This is a very dangerous drug and doctors that prescribe it are all too casual about the extreme negative effects.
      Increasing the "off label' applications of this awful drug is a terrible mistake.

      @HJ, people have known for years how to lose weight – diet and exercise, yet they are in too much of a need for instant gratification to be patient enough to let this method work. It is not a doctor's fault this drug was created, it was the drug companies because, ultimately, consumers are too lazy to work out and eat healthy.

    I've been taking Topamax for several years now for migraines. I will rarely experience some minor side effects such as tingling in my fingers, etc though it is impossible to tell if some of them (such as the cognitive issues) are from the drug or prior issues. Unfortunately, I have not seen any weight loss.

    I'm one of the lucky ones though. It's worked pretty well for me in stopping most, but not all, of my migraines without any of the major side effects that have caused others to choose to suffer the migraines rather than continue the medication (I believe I saw the number once...20% quit if I remember right.) You know that it has to be bad if you are willing to go back to frequent severe migraines rather than continue taking the medication. I don't have an issue with them using this for weight loss, but they need to be responsible about it and make sure that people know what they are potentially getting themselves into.

    I c this has devolved from serious 2 silly. Topamax for migraines gave me that same tingling in many joints. Not bothersome but felt like someone cleaning my veins! I built up from 10 to 100 mg a day. It didnt help the migraines but did turn me off even basic nutrition. Severe nausea. I wasn't interested in losing weight but lost 30 lbs in 2 mos. I also had the mood changes. It is possible the med can be tweaked. I've had migraines for many years and this is the most intense drug I have ever tried. My body felt like it was fighting a battle. Re fasting, there is medical evidence that occasional fasting is very good for the body.

    I am sorry but if someone needs to have a doctor prescribe them drugs to lose weight that is straight sad and lazy and that obesity is a "disease" is a bunch of BS! If you have to go through such extreme measures to loose weight then you have other problems you need to figure out first. I was morbidly obese in November 2010 and since then have lost 56 lbs thanks to a weight management program I went on called MonaVie RVL. In order to loose weight healthy you need to have plenty of nutrition for your body like RVL did for me, not be prescribed some drug. . . I am sorry but this is jut sad what our pharmaceutical industry and doctors have came to the conclusion of doing. Try getting out and exercise just 15 mins a day. It will do your body wonders!     The secret is to eat in moderation foods that are good for you and to get exercise. Unfortunately, people think pills can take care of everything from weight control to libido. Maybe they can help, but the person with the weight problem has to take responsibility for his/her condition. Stop driving through for fast foods – have a banana, chicken sandwich and water instead and go for a walk or bike ride after that.

    Really it is very simple. It just takes a kick in the ass to get going on it.

    Given the choice between debilitating migraines and all of Topamax's side effects (tingling, panic attacks, general indifference, foggy cognitive ability bordering on feeling stupid, and mid-day sleepiness), I'd rather all of the side effects than a migraine. Even if Topamax made me grow a tail and bark at the moon I'd still take it for my migraines given its benefits for me but, there is absolutely no way I would ever take Topamax for weight-loss.

    If everyone would read labels and please stop buying any food made with all soy forms as well as stop buying products with high fructose corn syrup their weight issues would begin to resolve in many cases. These two "food" items are very often found together in very many processed foods. From what I have read, the soy acts a inhibitor of the thyroid function (metabolic regulation) and the high fructose corn syrup when ingested is immediately stored in the body in the fat cells (like a toxin) and probably takes all other food eaten with it to fat also.

    To all you naysayers out there – That pic could be my body and I've gotten that way by eating a 1200 to 1300 calorie a day healthy diet and exercising 4 times a week for the last 10 years. Still I've ended up gaining 50 lbs because of METABOLISM PROBLEMS!!!! I've been seeing an endocrinologist for the past 2 years and my metabolism has improved, so no more gaining. But my Drs. have told me to be happy with being healthy & that I may never be able to actually lose the extra weight. So something like this could be a god-send to people like me.

    Pete's right. I totally threw up like 2 pounds worth of food after seeing the picture for this article. See now if only they could come up with a pill that would help obese people feel the shame associated with their gluttonous appearance and lifestyle then maybe they would feel as disgusted with themselves as we feel by having to look at them. Sorry to make it seem like I'm hatin' on fat people, but seriously, how can you let yourself get that OUT OF CONTROL? If it were a strung-out looking meth addict a similar level of public intervention might be needed. But overweight people think too highly of themselves to allow themselves to be placed in the same category as other addicts in desperate need of help. Maybe if they'd stop letting their ego and unrealistically high self-esteem get in the way of them getting some help, then they wouldn't be in this position. I think one of the WORST movements to come out of the last decade was the empowerment of fat people and them trying to convince the public that "big is beautiful" and that we should embrace them for how they are instead of trying to convince them to change (and lose weight and get HEALTHY). But that's like a coalition of meth addicts joining together to convince the public that their rotted teeth, pimpled, gaunt face, and stringy hair is beautiful and that the public should be ashamed of themselves for trying to convince the meth addicts to change, get healthy, or get help. Fat people need to get over themselves and GET HEALTHY and the public needs to wake up and see that OBESITY IS NOT BEAUTIFUL!

    Nice idea Pete! The strategic positioning of this photo on fridges and cupboards across the nation could prevent thousands of would-be blimps from stuffing their faces with Cheez-its and Twinkies. In fact, placing this photo on junk food packaging could deter fatties in the same way that health warnings on cigarette packages supposedly work to deter smokers. Pete, far from being a "moron" or a "goofball", I think you're on to something. Great contribution!

    OBESITY IS NOT BEAUTIFUL! I understand that overweight people have a difficult struggle to get themselves healthy again. It is probably on par with how difficult it is for drug addicts to get sober. But there is NO excuse for obese people to go around saying that big is beautiful and they like themselves this way. That's the same excuse drug addicts use – that they don't NEED to change. But just because the "drug" that obese people are addicted to is legal (food) doesn't make the effects of it any less devastating than the effects of illegal drug abuse. Society needs to wake up and stop AFFIRMING overweight people's position that they don't need to change. They DO Need to change – they're sick, they are NOT HEALTHY, and they need help. Oh, and this post along with my first post are the only two written by me. The other person posing with my screen name of "Bulimic Now After Seeing that Picture" and posting short comments about gays is not me. But I do strongly support gay rights and the rights of gay to marry, just in case anyone was curious.

    Lori & Laurent Gbago, I think we have a saboteur on the comment board posing as other posters. I think the mean reply you got from "Pete" was not the same "Pete" that started the comment string. I know a couple of comments have been posted using my screen name that were not from me. So just keep that in mind. It's probably just an obese person sitting at home on their couch/bed doing the only kind of activity they can do without breaking a sweat – typing on a keyboard. Worse yet, it's probably OUR tax dollars that are paying this person's disability check so they can sit at home and be fat and unproductive for the rest of their life. But yeah, when we take to the comment board to openly discuss such things all of these other people come out against us like WE'RE the bad, mean people! Fat people just do not like to be called out on their out of control lifestyle. But hey – IF THE TRUTH HURTS.....!

    Actually, pasting photos like the one in the article onto your fridge has been shown to have the opposite effect. People look at it and say to themselves (I am still okay, because I am not as fat at the person in the photo. What has shown to be effective instead, is to post a photo of a hot, in shape woman or man in a swimsuit. You will feel fat when you compare yourself to the photo, giving you something to strive for.

    I took Topamax for migraines, and I was pleased when I quickly lost 10 pounds (within six weeks of starting it). The side effects of the drug are terrible, however. Not least among them is the brain fog it causes. The drug literally makes you stupid. It also causes dizziness, muscle spasms, tinnitus, sleep disturbances, nerve changes such as leg pain or numbness. One of the reasons it makes you lose weight is it changes your sense of smell - and therefore taste. While I was on it i couldn't bear strong food odors, like onions or garlic. Savory foods tasted terrible to me, so I ate sweet foods or mild carbohydrates. And I gained back most of the weight. It's a terrible drug.

    Topamax can be a dangerous drug.Topamax really??? wow my Docter just about refused to put me on that for Mental conditions because of the side effects.I lost 120pds without any medications it's hard to lose weight.I am beyond proud of myself but I really wish I could have that skin removal procedure it sucks but it's worth it for the health benefit's.

    If this drug's potential to harm pregnant women and produce birth defects is proven even in the least, it should not be introduced into the market place. With the ever growing problem of pharmaceutical residue turning up in our drinking water systems, I think it would be unwise to introduce a drug with the kind of side effects described into the system. When you consider how widespread its use would likely be if it is an effective weight loss pill, it would not be surprising for it to turn up in the drinking water. While the fight against obesity is an important one, there are better ways of combating it than relying on a medication that could find its way into the bodies of people for whom it is not prescribed.

    Yeah, just read the side effects on Topamax...patients often call it "Dope-a-max" and there's a reason for that. Don't trust anything the pharmaceutical companies tell you. How many times have they come out with a miracle drug only to find out a year later that it's got all kinds of problems they didn't warn us about, especially if the drug is prescribed for something it wasn't originally intended to treat (like Topamax)? Miracle drug and a year later it's commercials with shady attorneys asking, "Have you or a loved one been injured by ____?"

    I notice someone who took Topamax for migraines and says it worked great.
    I have also taken Topamax.
    It does take away migraines.
    But for too many people the side effects of this brain altering drug are too unpleasant and dangerous to consider.
    Topamax is not going to be used for weight loss because the street name for it is STUPAMAX.
    Unless you have a crippling seizure disorder, do not consider topamax.

    I had heard that Topamax can be very effective in helping with weight loss, so I did considerable checking around the web, because I had planned to discuss this with my doctor.

    I found that slightly over 50 percent of people taking Topomax have side effects. Some of these side effects are rather mild in some people, and for them, they are worth the trade-off.

    I did read, over and over, that for some people the side effect is significant (sometimes irreversible) hair loss.

    That did it for me. If you are considering Topomax, you owe it to yourself to check this topic out carefully. You could end up seeing your scalp.

    I personally take 200mg of Topamax 2 x's a day for something called Hemiplegic Migraines. They are not fun at all! . I also take Thyroid meds and have been doing this combination for the last 6yrs. The side effects are not as bad as everyone seems to think they are for Topamax. Do you loose weight? Yes, but only till your body gets use to the doseage you are taking. The only true way to loose weight is thru diet and excersize. I'm not to hip on taking any type of drug, but unfornately, I have too. combining these 2 "super" drugs and then selling them for a diet pill is simply ludicrous.

    This drug is a little scary. Phenteramine is generally regarded as safe-ish, but it is half of the older drug Fen-Phen, which was pulled from the market by the FDA in the 90s. Topamax is also a drug that I would not voluntarily take. But that being said, if someone is extremely obese, the benefits might outweigh the risks.
